Belt Lipectomy (360 Degree Body Lift)

A belt lipectomy removes a continuous band of loose skin and fat from around the waist: the lower abdomen at the front, the flanks at the sides and the lower back behind. It suits people whose loose skin carries on around the trunk after major weight loss, and it leaves a scar that circles the body at about belt level.

It is also called a 360 degree body lift. Most people who need it have already looked at a tummy tuck and found that it only describes the front of their problem. What is a belt lipectomy?

It is an operation that removes excess skin and fat from around the lower trunk to improve its shape. StatPearls describes the removal as running in a circle around the abdomen, flanks and back.

The word belt describes the shape of what comes away. The skin is removed as one band, like a belt, and the edges above and below are brought together. The Cleveland Clinic describes the incision as wrapping a full 360 degrees around the body.

What does a belt lipectomy remove that a tummy tuck does not?

The back half. A tummy tuck treats the front of the abdomen. A belt lipectomy continues the same removal around the flanks and lower back, and tightens the buttock area behind.

The two are not alternatives. StatPearls notes that a belt lipectomy often includes a formal tummy tuck within it, with the navel repositioned and the abdominal muscles tightened. So the real question is whether a front-only operation would leave work undone on your body.

Who does a belt lipectomy suit?

People whose loose skin does not stop at the sides. These are the signs that point towards it:

• Loose skin folds wrap right around your waist
• Your tummy, hips and lower back all sag after weight loss
• The skin chafes or develops rashes in the folds
• Clothes do not sit well because of hanging skin at the sides and back
• Your weight has been stable for several months

It does not suit someone whose excess is confined to the front. In that case a tummy tuck is the correct operation, and the larger one adds a scar across the back without removing anything. Which pattern you have is settled by examination while you are standing, not from a photograph.

Is a belt lipectomy the same as a lower body lift?

The names are often used for the same thing. StatPearls lists circumferential body lift, lower body lift and torsoplasty as other names for a belt lipectomy.

In practice the difference is how far the lift is carried. A belt lipectomy describes the band removed at the waist. A lower body lift is planned so that the buttocks and outer thighs are lifted as well. How is a belt lipectomy carried out?

Under general anaesthetic, in sections, with your position changed between them. No single position gives access to the whole circle.

• Marking while standing. The plan is drawn on your skin while you are upright, because skin falls differently standing than lying flat.
• Three positions. StatPearls describes the operation as often needing three position changes, working on each side and then on the front.
• Removal and tightening. The band of skin and fat is removed. The Cleveland Clinic notes that the abdominal muscles may be tightened at the same time.
• Closure. The wound is closed in layers. Drains are often placed, although StatPearls describes them as optional when internal tension stitches are used.

Because the positioning is designed around the trunk, a belt lipectomy is not an operation that is casually extended to the arms or thighs on the same day.

Where does the belt lipectomy scar sit?

All the way around, in one continuous line at about belt level. It is planned to sit low at the front and where underwear usually sits at the back.

It can be hidden by clothing. It cannot be made invisible. The Cleveland Clinic states that scars are expected on the waist and lower back, and that they fade with time and never disappear completely.

A tummy tuck scar is somewhere you rarely see. A belt lipectomy scar is visible from behind, in a mirror and at a pool. Ask to be shown where the line will sit on your own body before you decide.

What is belt lipectomy recovery like week by week?

The main difference from a tummy tuck is that you cannot rest on the wound, because it is behind you as well as in front. Your own timeline is set at follow up. These are the stages the published guidance describes.

• First days. Early walking is encouraged and an abdominal binder is worn. The Cleveland Clinic notes that much of early recovery is spent sitting in a comfortable chair instead of lying in bed, to reduce pressure on the incision.
• Weeks 1 to 3. No bending at the waist and no strain on the closure. Most people rest for around three weeks before returning to light routines.
• Weeks 3 to 6. Light activity builds up. The support garment stays on. StatPearls notes that compression is often worn for several weeks to months.
• About week 6. The Cleveland Clinic advises no exercise or heavy lifting for approximately six weeks.
• Months 3 to 12. Swelling settles slowly. The Cleveland Clinic notes that skin and tissue can take up to a year or more to settle into their final shape.

What are the risks of a belt lipectomy?

The risks named by StatPearls and the Cleveland Clinic are fluid collection under the skin (seroma), wound separation, infection, bleeding or a blood collection (haematoma), loss of a patch of skin, blood clots in the legs or lungs, numbness, persistent swelling, uneven contour, and skin that loosens again.

Two things make them more likely. The Cleveland Clinic notes that complications are more common than with other plastic surgery because the trunk is always moving. StatPearls notes that complication rates rise as body mass index rises. Both are reasons the preparation is strict.

When should a belt lipectomy wait?

• While you are still losing weight. StatPearls advises waiting until weight loss goals have been reached and held for six months to a year.
• If you plan a pregnancy. The same source advises waiting until childbearing is complete, because the skin can stretch again.
• If you smoke. Patients are usually asked to stop at least a month before surgery.
• Until nutrition is corrected. Deficiencies can follow bariatric surgery and are treated first.
